Certificate of Pension Membership

The DPA permits out-of-state residents to file for a Certificate of Domestic Partnership when at least one of the partners is a member of a New Jersey State-administered pension system (the partners must also meet all other requirements of the law for entering into a domestic partnership and share the same place of residence in the out-of-state jurisdiction).

Applicants using this form to certify pension membership should have their employer complete and certify the required information, and submit the completed form to a Local Registrar along with their Affidavit of Domestic Partnership. Download Form CP-0696-0504
